Wolfbrother mentioned Survival and Austere Medicine. You folks may be interested to know third edition is due out later this week. :-)  Slightly larger than last time, 614 pages vs. I believe it was 212. As before the pdf version will be free, and the hard copies sold without royalties tacked on.
