This is a very good explanation (With lots of pictures) of making your own Bacon (BACON !!!)

Pork belly can be found at real butcher shops.
Buy local pork if you worry about the source of your meats.
More is better.

You will need equipment, spices and time:

Smoker
Seal-able bags
Ground pepper (optional)
Salt
Salt Peter (Sodium Nitrite) or "Prague Salt"

Final warning, this is food porn at its finest.
